The history is detailed and the time you invest in thoughtfully answering the questions will contribute to the productivity of this initial discussion. Please fill out the packet provided at the bottom of this page. You may either fill it out electronically and submit it immedietly (please also print it and bring a hard copy to your appointment), or you may print it and fill it out by hand. You are invited to bring any laboratory reports from the last six months to avoid duplication of laboratory evaluations.
