In preparation for your initial visit, please complete the new patient history packet prior to your appointment. The history is detailed and the time you invest in thoughtfully answering the questions will contribute to the productivity of this initial discussion.
Please fill out the New Patient Health History Packet provided below. You may either fill it out electronically and submit it immedietly (please also print it and bring a hard copy to your appointment), or you may print it and fill it out by hand. You are invited to bring any laboratory reports from the last six months to avoid duplication of laboratory evaluations.
